The qualified team at Achieve Care offers comprehensive assistance required for higher education and employment. Our support is designed to assist participants to gain the necessary qualifications and build their skills & capacity which further helps them to participate in employment or to find and maintain appropriate employment in the open market or in supported employment.
Our specialists assist a participant in transition to school or to post-school options (e.g. to further education, training, or employment) and building their capacity for independent living and self-care, developing social and communication skills, developing specialist behavior management plans, and enabling the participant to travel independently to their place of education.
Our assistance with daily tasks/shared living focuses on developing and enhancing domestic skills that allow participants to live autonomously. It can also be delivered within shared living such as units or host family arrangements. It may include support for self-care, and assist to arrange a residency for living independently.
Achieve Care connects the participants to various group programs on offer with options targeted to a variety of interests and new activities in the community. These activities result in the development of natural friendships and peer networks, better knowledge of the community and opportunities it presents, improved participation in activities, a sense of belonging, enhanced self-esteem, independent living, etc.
Our team offers community participation services when a participant is in out-of-home care and requires support specific to the participant’s disability that is additional to the needs of children of similar ages, including assistance with daily personal activities, aids and equipment, community participation, and home modifications, etc.
Community Nursing Care is provided to participants who have complex individual support needs that are best met by someone who has medical training and experience, to meet the Participant’s complex health needs. This service includes day-to-day management of medications, complex wound management, complex bowel care and enteral nutrition, etc.
We offer individualized assistance that may be specific to a participant’s disability (or developmental delay) and associated with the functional impact on the participant’s activities of daily living.
At Achieve Care, we offer training and support to our clients in using public transport where these needs arise from a participant’s functional impairment. Our team also assists in travel & travel arrangements.
We work towards increasing the participant’s ability to live as autonomously as possible, including skills in daily life activities, communication, and social skills, problem-solving, and learning basic life skills.
